San Mateo Medical Center (SMMC) is seeking a highly-qualified Social Worker III / Discharge Planner to provide highly skilled, intensive casework services to individuals and their families, and provide casework consultation within SMMC and to outside agencies. The incumbent will have the opportunity to work as a floater between Medical-Surgical Unit, Skilled Nursing and the Emergency Room, and work jointly with Case Managers, supporting Emergency Department staff in allocating necessary resources to avoid unnecessary hospital admissions. The Social Worker III/Discharge Planner will report to the Resource Management Department with a primary focus on intensive care and emergency.
QUALIFICATIONS
Education &
Experience:
Social Worker III - Satisfactory completion of 36 semester units of social welfare and/or behavioral science courses plus three years of case management experience comparable to a Social Worker II or a Master's Degree in Social Work with two years of experience performing duties comparable to Social Worker II or higher in San Mateo County.
